Steps to getting started
Step 1
Assessment
​
This is a starting point of your journey with us. It usually takes about 1.5 to 2 hours. Please allow enough time and bring the following:

ProviderOne card, if you have DSHS medical

Service Provider Information - Name, phone number, fax number (such as Attorney, Social Worker or Probation officers)​

Step 2
Follow up Appointment
If you decide to receive services with us then the next step is a follow up appointment. This appointment usually takes 30-60 minutes. During this time, you will:
